WORKPLACE-SPECIFIC TRAINING
A reminder is given to facility supervisory staff that to complete the educational process successful participants must return to work and be provided with workplace-specific training.
PERSONAL PROTECTIVE EQUIPMENT REQUIRED
Participants attending this course are required to supply and wear their own CSA-approved head protection, layered clothing, and appropriate footwear suitable for an ice arena environment. No exceptions.
PARTICIPANT DISCLAIMER
As the registered participant, you are required to read and sign the following disclaimer in order for your registration to be processed: “I, the registered participant, agree to assume all risks, including travel to and from the program, and waive all rights and all liabilities for personal injury or property loss whether occurring prior to, during, or subsequent to the program whether caused by the ORFA’s negligence or otherwise, and for the consideration aforesaid, I covenant and agree not to make any claim or to commence or maintain any action or proceeding against them. And it is further agreed that the ORFA does not by the payment aforesaid or otherwise admit any liability to me and liability is in fact denied. As a registered participant of an ORFA Inc. event, I acknowledge and agree to the mandatory use of head protection and all other personal protective equipment that may be required during my participation in any technical training course. Further, I hereby consent to my name, likeness, image, and voice being interviewed, recorded, photographed, filmed and taped for education, advertising, publicity, commercial or other ORFA business purposes and hereby release the ORFA Inc. and any of its associated or affiliated companies, their directors, officers, agents, employees and customers, and appointed advertising agencies, their directors, officers, agents and employees from all claims of every kind on account of such use.”
REFUND AND CANCELLATION POLICY
A complete refund, less a $75.00 administration fee, will be issued provided written notification of cancellation is received by the ORFA no later than 10 business days prior to the regional course start date or no later than than Friday, April 6, 2018 for Annual Professional Development Events. No refunds will be issued after this time period. Registrants will receive a full refund should the ORFA cancel a course. ORFA reserves the right to cancel any course. In the event of a cancellation, any costs incurred by the registrants will not be covered by the ORFA.
